Breast Cancer Early Detection Program - Continuum
of Care
Produced by the Central Coast Women's Cancer Consortium,
the Continuum of Care demonstrates the standard of
care every woman should receive when being evaluated
for breast cancer. Maintained by ICCS.

real-life problems in youth-oriented ways. YMN encourages
youth to communicate health messages through media.

quality of life for youth and families in our communities.
This collaboration sponsors community-based programs
and services targeted toward achieving progress on
priority issues identified by the TELLUS project and

Executive Summary and Regional Reports (2001)
Tellus/Diganos is a biannual countywide quality of
life assessment process that began in 1995. A broad
coalition of healthcare, county and city officials,
not-for-profit agencies, businesses, and foundations
came together to ask residents questions about their
view of life in the County. The goal: To find out
where quality of life needed improvement and to celebrate
progress from year to year when issues were successfully
addressed. Measuring trends every two years provided
a benchmark- a blueprint for our countywide community.

in Santa Cruz County, California. For each organization
the following are given: address, phone number,
services, and eligibility requirements. Updated
on an ongoing basis. May be searched by keyword,
