Page 8 | Page 9 | Page 10 | Page 11 | Page 12 | Page 13 | Page 14 Page 1: IntroductionThe Matrix Outcomes Model provides a set of complementary assessment, case management and evaluation tools, enabling health and human service providers to both empower and track individual, family, agency, and community outcomes over time and in a way that allows for the complexity of the challenges they face. These instruments were created through the efforts of federal, state and county agencies interested in achieving results accountablity. The Institute for Community Collaborative Studies, California State University Monterey Bay has evolved the Matrix Model with partners who continue to refine the language, structure, and guidelines for their use. The individual titles for these tools are the 1) Family Development Matrix, 2) Agency Development Matrix, and 3) Community Scaling Tool. The data provided by these instruments are useful in every aspect of program development, from needs assessment to measures of change to evaluation. When consolidated across agencies serving the same community, they become a powerful source of guidance for funders and policymakers, as well. The Institute for Community Collaborative Studies contracts with agencies to provide training and technical assistance. It is this group of stakeholders that is developing the evaluation tools for agencies seeking outcomes and building integrated services.
