CONTINUUM OF CARE TO
BREAST CANCER EARLY DETECTION


The Continuum of Care was developed by members of the Partnership, breast cancer survivors, and physicians. The concept mapping process was facilitated by Jerry Endres, ICCS Community Director, and Jon Sagen, principal of John Sagen and Associates. The goal of this Continuum of Care is to provide guidance to providers, as well as to the women who may be have questions about breast cancer. The Partnership, has agreed that this Continuum demonstrates the standard of care every woman should receive when being evaluated for breast cancer.
