80 - APPOINTMENT FOR VISIT#l - Patient is given appointment for first visit Patient is then mailed a program pamphlet containing information about breast self examination and information about the time, date and location of her appointment.
80.1 Utilize specially trained staff from community agencies to provide advocacy and patient follow-ups. Offer bilingual, interactive services available to ensure client comprehension. Provide simple street maps of the community with the location of other providers.
80.2 Develop correctly translated books, videos, and materials relating to diagnosis, treatment, nutrition, survival, psychological support and death, utilizing patient testimonials when possible. Include low literacy versions.
80.3 Provide breast self exam instructions in appropriate languages including mammogram frequency guidelines. Attempt to reduce fears by stressing importance of screening, rescreening and follow-up.
80.4 Provide complete and updated information on transportation systems and alternatives available in the community and assist in accessing transportation when needed.
80.5 Provide multi language counseling materials and referrals to underserved women above the poverty level.
