Step 1: Open your Web browser to: http://infoserver.csumb.edu
Step 2: Click on Planner Web and then on button: 'STUDENTS: Click to logon"
Step 3: At the prompt, enter your Banner User ID and Banner PIN
Step 4: At the Main Menu, click on "Service Learning Students"

If you have forgotten your Banner PIN, e-mail banner_pin@csumb.edu. Include your Student ID and birth date. If you are having problems logging, call Technology Support Services at 582-4357.

I. If you are enrolled in a liberal studies course, you must complete the following two forms:

1. Service Learning Placement Request Form
Completing this form is necessary in matching your interests and skills with the opportunities at the school placement sites. Choose placement sites from the list of schools that has been preapproved for your specific course. Once you have submitted your Placement Request Form, your choices will be reviewed and you will be assigned a final site. You are not guaranteed your first choice of schools. Your final assignment will be emailed to the e-mail address, which you provide on this form. If you have any questions, please call the Coordinator of Service Learning for Liberal Studies and Teacher Education at 582-4604

Due Date: by the end of the second week of classes

2. Student Evaluation of the Service Learning Experience
This form gives you the opportunity to evaluate the effectiveness of your Service Learning experience. The feedback that you provide on this form will assist the Service Learning Institute in improving the Service Learning program at CSUMB. Completing this form is mandatory for all Service Learning students.

Due Date: before your last day of class

II. All other students NOT enrolled in Liberal Studies courses must complete the following two forms:

1. Service Learning Placement Form
The Service Learning Placement Form provides the Service Learning Institute with accurate information about your Service Learning placement. On this form, you are required to (a) select your placement site from the list of preapproved Community Partners that appear in the drop down box; or (b) input the relevant information on your site, if it is a new Community Partner. This form becomes official record of your Service Learning placement for the semester.

Due Date: before your first official day of service at your placement site

2. Student Evaluation of the Service Learning Experience
This form gives you the opportunity to evaluate the effectiveness of your Service Learning experience. The feedback that you provide on this form will assist the Service Learning Institute in improving the Service Learning program at CSUMB. Completing this form is mandatory for all Service Learning students.
